What is an AI hallucination?
A hallucination is a confident but wrong answer from a language model, such as an invented figure, policy or citation. It is managed rather than eliminated: ground answers in your own data, require sources, route low-confidence cases to people and put consequential actions behind human approval.
Example
Instead of guessing a delivery date, the agent quotes the date from the carrier's tracking data, or says it does not know and escalates.
In one line
A confident but wrong output from a language model. Managed with grounding in real data, citations, confidence thresholds and human approval.
